The Social Service Department at Millcreek Community Hospital provides patients and their families with support and referral services to help cope with problems that arise during a patient’s illness.

The Social Service Department is comprised of dedicated and caring professionals with social work degrees. The Social Work Department will work closely with physicians, nurses, and therapists to help patients function as best as they can in their environment.
Our staff can provide patients with information on resources in the community and emotional support during their road to recovery. We also offer assistance and guidance to meet many of the patients needs such as: discharge planning, medical power of attorney information, advance directive information, domestic violence, child/elder protective services, transportation needs, notary services and prescription drug information.
