SOCIAL WORKER - MILLCREEK MANOR
Millcreek Geriatric Education and Care Center (Millcreek Manor) is seeking a Full Time
Social Worker. Strong communication, organization and time management skills necessary; working
with staff, patients, residents and families. Responsibilities include developing a
comprehensive care plan for each resident to include medical, nursing and psychosocial needs, MDS
3.0 assessments and coordinating discharge plans. Bachelor's Degree in Social Work or Human
Services required including one year supervised Social Work experience in a health care
setting.
